Student Health Insurance Program at Jacksonville University

Jacksonville University requires all full-time traditional undergraduate students to carry a blanket accident and health insurance policy. Markel Insurance Company has been chosen to underwrite and administer a Student Health Insurance Program specifically designed for JU students.  You may choose to maintain health insurance independently or you may purchase this coverage available through the University.  If a student has local coverage under their existing plan and does not want JU's coverage they must submit a waiver with a copy of their insurance card (front & back).  Students without complete and proper waivers will be automatically enrolled in the Student Health Insurance Program.  The insurance cost will be added to the student account.

If you have other coverage, please consider the following before making your decision to complete the waiver form:

• Review your current health plan, if you are a member of an HMO or PPO; please be certain of service available in the Jacksonville, FL area.  Many plans have geographic limitations.
• Your family policy may not cover you after you reach a certain age.  Plan ahead.  Don't wait until this occurs and you find yourself without protection.
• Given the above, please consider the low cost of this plan for 12 months protection.  Your participation will provide you with coverage that can fill the gaps of primary coverage or financial assistance at a time when you need it the most.

Claims Procedure

1. Complete a claim form, which is available online here or at the Controller’s Office. 
2. The claim form must be completed and signed.  Attach all itemized medical and hospital bill. Itemized bills must be furnished with the claim form within 90 days from the date of loss.
3. Questions should be referred to the claims Administrator listed below.

Coverage is for full time traditional undergraduates only for 2005-2006.  Beginning with the 2006-2007 academic year, coverage is extended to students carrying a minimum of 9 credit hours and under the age of 25.

Premium Cost for Academic Year 2005-2006

$540 For all students enrolled in Fall 2005 – coverage is Aug 1 – July 31
$335 For all new students enrolling in Spring 2006 – coverage is Jan 1 – July 31
$185 For all new students enrolling in Summer 2006 – coverage is May 1 – July 31

Premium Cost for Academic Year 2006-2007

$555 For all students enrolled in Fall 2006 – coverage is Aug 1 – July 31
$345 For all new students enrolling in Spring 2006 – coverage is Jan 1 – July 31
$190 For all new students enrolling in Summer 2006 – coverage is May 1 – July 31

Student Health Insurance ID cards will be mailed to the student’s local address by Markel Insurance Company.

Special note for International Students
Waivers will only be accepted if your insurance coverage is based in the United States and the policy must be printed in English.

Special note for Active Duty Military Students
If you are non-NROTC Active Duty you will need to provide a completed waiver form and a letter from your CO, which states that you have full health and accident coverage.

Special note for Military Dependent Students

If you have Tricare Prime, a copy of your insurance card is still a requirement.  If you do not have your card, please visit www.humana-military.com to access your card online.

If you have Tricare Standard or Extra, your military ID card should not be photocopied.  Please submit your waiver in person to someone in the Controller's Office and show them your ID.  They will then mark your waiver as showing proof.
